Complex Guidance Trajectory Coach Casemanager

North HollandHybrid34 hours/week<3months

Are you looking for a role where you combine substantive guidance with direction, coaching, and oversight?

  • Do you make a difference for clients with complex issues and support colleagues in their work?
  • As a Casemanager, you are the linchpin in complex care trajectories.
  • You connect clients, personal supervisors, and chain partners, ensuring everyone knows what is needed, when, and by whom.
  • You maintain the common thread, provide structure, and ensure that care progresses instead of getting stuck.
  • You will work within an Ambulatory Specialist Team of a healthcare organization in an urban environment.
  • The team provides temporary, intensive ambulatory guidance to independently living clients with complex support needs.
  • The trajectories are as short as possible and as long as necessary, varied and focused on recovery, skill acquisition, providing tools, and stabilization.
  • Customization is important in this context.
  • Within the team, there is room for consultation, reflection, and personal input.
  • You work in a dynamic and culturally diverse environment.
  • The work is intensive and often complex, but provides great satisfaction as you visibly contribute to stability and perspective in clients' lives and the professional development of colleagues.

What you do

  • Coaching personal supervisors: you support colleagues in their daily work, contribute to complex case discussions, and assist in drafting and adjusting guidance and care plans.
  • Intake and case management: you process incoming applications and support intake interviews. Together with the personal supervisor, you determine the most suitable guidance for the client. In complex situations, you, as a casemanager, are always involved.
  • Trajectory monitoring and direction: you monitor the progress of care trajectories, identify bottlenecks, and adjust where necessary, so that the client can regain self-direction.
  • Collaboration and network contacts: you actively maintain contact with neighborhood teams, aid organizations, and other chain partners to ensure coherent and effective support.
  • Own caseload: you also guide clients yourself (maximum 50%), to stay closely connected to practice and the target group.
  • Signaling and advising: you analyze situations, identify bottlenecks within the team, and contribute to a safe working environment within the team.
  • Quality of care: you are co-responsible for the quality of care, propose improvements, and help implement them in the teams.

What they ask

  • HBO diploma in a social-agogic field (e.g., Social Work).
  • Experience with ambulatory work within mental health care (GGZ) and care for people with disabilities.
  • Experience with coordination and the ability to act and think in a process-oriented manner.
  • Strong in collaboration, networking, and taking direction.
  • Careful and honest handling of confidential information.
  • Decisive, results-oriented, and dares to take responsibility and look with a critical eye.
  • Experience with crisis intervention, mediation, and aggression management.
