What you will do. As a supervisor or supportive supervisor:
- Coach, motivator, care provider, and confidant in one
- Encourage clients to take steps forward and guide them towards an independent life
- Work based on Triple-C: relationship-oriented, development-oriented, strong together
- Do what is possible yourself, provide support where needed, and build a trusting relationship
- Collaborate in a dedicated multidisciplinary team
As a service coordinator (DVC) / care coordinator:
- Connecting link between client, team, and care network
- Draw up a clear support plan and monitor the quality of care
- Actively assist at the residence, combining coordinating tasks with care services
- Coach and content driver: stimulate ownership, provide structure, maintain overview
- Collaborate with a multidisciplinary team for a safe and stimulating care environment
Profile
For (supportive) supervisor:
- Care-related Mbo-3, Mbo-4, or Hbo diploma
- Career changers with a warm heart for care
For DVC:
- Care-related Hbo diploma, or
- Level 4 with Hbo working and thinking level and relevant work experience

- Flexible deployment, with varying shifts on weekends and evenings

- Genuine interest and attention for clients, their loved ones, and your colleagues
- Professional and appreciative, contributing to a meaningful life for clients
Practical aspects.
- You work according to a varying schedule, with morning, evening, weekend, and holiday shifts, always together with a dedicated team
- Opportunities to get acquainted with multiple locations through trial shifts
- You work with direct colleagues and a multidisciplinary network such as behavioral experts, care coordinators, Arts-VG, and other disciplines
- A Certificate of Good Conduct (VOG) is required
