Personal supervisor LVB+

At a care facility, you support (young) adults with a mild intellectual disability (LVB+) and complex behavioral issues. You look beyond behavior alone and help residents step by step in building as independent and meaningful a life as possible. You maintain an overview, connect, and coordinate care around residents. You are the fixed point of contact for residents, family, and involved professionals. The facility is a modern and brand-new building in a quiet, green neighborhood.

What you do

As a personal supervisor, you combine daily guidance with case management and care coordination. You maintain an overview of the residents' support process and ensure that goals, agreements, and guidance align well.

  • Managing your own caseload, drawing up, evaluating, and adjusting guidance plans, and coordinating care around residents.
  • Maintaining contact with family, relatives, therapists, and other collaboration partners, acting as the fixed point of contact to safeguard connections.
  • Providing daily guidance and support, creating structure, clarity, and predictability, and supporting residents in their personal goals.
  • Signaling changes in behavior or situation and acting adequately to ensure a safe and stable living environment.
  • Collaborating intensively with supervisors, behavioral experts, and other disciplines, supporting and coaching colleagues from your expertise.
  • Actively contributing to the further development of the facility and continuously improving the quality of care.

What they ask

  • A completed MBO 4 or HBO degree in healthcare (HBO work and thinking level; MBO 4 diploma requires an assessment).
  • Experience with or affinity for people with a mild intellectual disability and complex support needs.
  • Strong organizational skills and the ability to maintain an overview.
  • Resilience, self-reflection, and the ability to relativize, good communication skills.
  • A positive and professional attitude and the desire for continuous personal and professional development.
  • The motivation to build the best possible care together with colleagues.
  • Flexible availability for day, evening, and weekend shifts (no night or sleep shifts).
