Youth and Family Coach

You support families in reducing stress factors and strengthening the family unit, so they regain control over their situation and their lives. You work from one of the district teams of an urban municipality.
What you will do.
- Provide basic assistance from the district team, with responsibility for your own caseload.
- Conduct home visits and, together with the family, identify what is needed to improve the situation.
- Develop a care plan and involve the child's network (e.g., family members and friends) and the district network (e.g., schools and youth and family centers).
- Offer basic parenting support and advice/assistance in various areas of life, such as meaning, housing, work, finances, social relationships, and mental health.
- Engage specialized or more intensive/long-term care if basic assistance provided by the district teams is insufficient.
- Act as a case manager, ensuring the family receives an indication for appropriate care and remaining involved with the family.
- Be responsible for the progress of the care, including periodic evaluation moments.
- Work within a multidisciplinary district team where colleagues from different parent organizations collaborate closely.
- Work intensively with various chain partners, both within and outside the district.
- Ensure positive stimulation, structure, expression of emotions, guidance of school activities, and language stimulation.
Profile
- You enjoy a significant challenge and bring knowledge and experience in supporting families with multiple and complex issues.
- You are independent, flexible, and creative in thinking about providing customized solutions.
What they ask
- An HBO diploma (e.g., Social Work, MWD, SPH, or Pedagogy).
- An SKJ registration. If you do not have this, you can obtain it in the short term through a practical program, in addition to having several years of work experience in care.
- Knowledge of and insight into the range of assistance and various care methodologies.
Preferences.
- Preferably, knowledge of the Youth Act, the guidelines for youth professionals, and the reporting code.
Practical
- You work on-site and conduct home visits.
- The district teams are spread across an urban environment.
- A district team consists of 2-8 colleagues for parenting questions, supplemented by colleagues from other parent organizations focusing on youth with disabilities or adult psychiatry.
- Each district team is substantively supported by a behavioral scientist.
