The client is a government organization responsible for the governance and implementation of various public services. The work package includes, among other things, public affairs, permit issuance, management of public spaces, and programs related to the living environment, housing, mobility, youth, participation, and social support.
As a social worker, you will conduct home visits to residents who have registered with the Area Team for Protected Living and for questions regarding Wmo guidance, day activities, and where possible, roles and transport. You will clarify and analyze requests for help, create and mobilize the network of those involved and chain partners. Together, you will draw up a clear plan of action, addressing the request for help and involving all relevant areas of life.
You will also provide information and advice in an understandable manner.

- Conduct investigations and process applications for Protected Living
- Handle and process Wmo applications specifically in the areas of guidance, day activities, and protected living
- Clarify and analyze requests for help and mobilize the network and chain partners
- Together with stakeholders, draw up a clear plan of action that addresses the request for help and involves all relevant areas of life
- Provide understandable information and advice to residents
Profile
As a social worker, you will visit residents at home who have registered for Protected Living and for questions regarding Wmo guidance, day activities, roles, and transport. You will investigate and process applications for Protected Living and other Wmo provisions as needed.
