Dementia Case Manager
A healthcare organization is there for people who need intensive, specialized care. For people who, due to illness, old age, or a combination of the two, can no longer live independently or temporarily need support. We help clients continue their lives as they wish, despite their health problems. Based on the idea that 'the whole life counts', we aim to provide loving, personal, and high-quality care to vulnerable people with complex care needs.
In this role, you will work in a team that provides support to clients with a Full Home Care Package (VPT) indication in the region. The teams consist of nurses, caregivers, and hospitality staff. The goal is to give clients as much control as possible and enable them to continue living independently at home.
What you do
- As a case manager, you guide clients with dementia living at home and their informal caregivers.
- You map out the client's situation and needs and jointly determine what care and support are required.
- You coordinate care, maintain contact with all involved parties, and serve as a trusted point of contact for the client and their loved ones.
- You offer a listening ear, provide advice and information, and help clients and informal caregivers find appropriate support.
- In doing so, you look beyond the care question alone. What does someone need to continue living daily life as pleasantly as possible? And what is needed for this in the home situation?
- You work closely with other healthcare professionals and involved organizations.
- Together with colleagues and collaboration partners, you build a strong connection between care and well-being, both within and outside the organization.
- You contribute ideas for new possibilities and to the development of the team.
- Together, you ensure that clients can continue to live safely and comfortably at home for as long as possible.
- If the situation changes and living at home is no longer possible? You then guide the client and their loved ones in finding a suitable living arrangement.
- In this way, you contribute to the organization's vision: supporting people for as long as possible to live in their own way and in their familiar environment.
Profile
- You are a committed and enthusiastic professional who easily connects with clients and informal caregivers.
- With your warm demeanor, you build trust and offer a listening ear.
- You take initiative, think in solutions, and maintain an overview, even when the situation is complex.
- You enjoy collaborating with other healthcare professionals and always consider what a client needs.
What they ask
- Possess a higher professional education (hbo) diploma, for example, hbo-nursing, Social Work, or Applied Psychology, supplemented with training as a dementia case manager (or be willing to follow this training).
- Have current knowledge of home care, or be willing to deepen your knowledge in this area.
- Have an affinity for people with dementia and their informal caregivers.
- Be able to work independently, organize well, and connect various stakeholders.
Practical
- Salary according to FWG55 from the VVT collective labor agreement.
- End-of-year bonus of 8.33% and 8% holiday pay.
- Opportunity to develop yourself and truly make a difference for clients!
- A VOG (certificate of good conduct, reimbursed by the organization) and diploma check are part of the process. A reference check may also be conducted.
