As a healthcare consultant within a healthcare organization, you will have a supportive, guiding, and coordinating role. The goal is to monitor and improve the quality and continuity of care for clients. The organization provides live-in care to clients of all ages in their familiar home environment, delivered by live-in care providers. This responds to the societal trend of enabling clients to stay at home for as long as possible.
What you will do.
- Conduct intake interviews with clients and/or their families to determine care needs.
- Together with the client and/or family, draw up a care plan and ensure the completion of the digital care file.
- Select a suitable care provider, in consultation with the client and/or family and back-office colleagues.
- Maintain contact with the client and/or family and regularly evaluate the agreed services with the client and/or family and the care provider.
- Support and coach the care provider in the execution of their duties.
- Provide, solicited and unsolicited, opinions and advice on current developments in healthcare to your supervisor.
Profile
- Independent and proactive management of your own client portfolio, with support from colleagues.
- Socially skilled, with a certain degree of persuasiveness in contact with clients, family, and care providers.
- Clear communication, both oral and written.
- Solution-oriented, stress-resistant, and creative.
- Caring and enjoying interaction with (sometimes vulnerable) clients.
- Punctual and meticulous in adhering to agreements and client administration.
- No 9-to-5 mentality.
- Speaks Dutch, English, and/or German (working language of foreign care providers).
What they ask
- Relevant healthcare education at MBO or HBO level, preferably a nurse or VIG (care assistant) qualification.
- In possession of a driving license.
- Computer literate.
