As a Social Worker, you provide service and first-line assistance to chronically ill individuals, people with disabilities, and care-dependent elderly living at home.
What you do
- Inform and provide personalized advice to individuals within the target group about the range of services and facilities in home care to ensure maximum utilization of their rights and to promote their well-being and self-reliance.
- Help find solutions, assist, and refer to the appropriate authorities.
- In severe home care situations, you offer care guidance.
- You organize and support home care tailored to individual needs in collaboration with internal and external professional partners, volunteers, and informal caregivers (integral approach).
- Correct administrative follow-up of client files is also part of your duties.
- These services and assistance are provided through various contact forms: appointments, home visits, telephone, and email.
- In addition to answering questions, you also proactively establish contacts.
- You work closely with your colleagues, the organization's other services, and other (home care) services in the region.
- You support the team in a first-line zone.
Profile
- You possess one of the following diplomas:
- graduate, bachelor's, or master's in social work;
- bachelor's in social healthcare;
- bachelor's in social nursing or a nurse specialized in social healthcare (with attention and interest in social legislation or willingness to learn it);
- graduate or bachelor's in family sciences.
- You have a client-oriented and service-minded attitude.
- You have a strong personality and maintain a good balance between empathetic assistance and professional boundaries.
- You are skilled in clarifying questions and quickly grasp the actual need for help.
- You are excellent at planning and organizing and can manage your work independently.
- You are a driven team player with smooth communication skills.
- Experience in home care and affinity with the field are a plus but not a requirement. Motivated starters are also eligible.
- You possess a B driving license and a car.
- You can present a clear criminal record extract (model 1) no older than 3 months for inspection.
