A dynamic group of approximately 25 professionals provides elderly and health law services to citizens in their own homes. The digital team is a multidisciplinary team consisting of nurses and social and health assistants, providing elderly and health law services via screen and telephone. The team focuses on prevention, rehabilitation, and supporting citizens' independence through digital solutions. Daily collaboration with all home and nursing care groups is essential. Both areas focus on:

What you do
As a social and health assistant, you will, among other things:
- Perform care, welfare, rehabilitation, and nursing tasks
- Work rehabilitatively based on the citizen's resources and goals
- Observe, assess, and act on changes in the citizen's health status
- Manage digital citizen contacts via screen and telephone
- Use your clinical judgment to assess citizens' needs, both physically and digitally
- Document professional efforts and observations in a care system
- Collaborate closely with nurses, therapists, doctors, a dementia team, and other partners
- Contribute to the development of new workflows and digital solutions
- Participate in coherent and coordinated citizen processes
