As a practice supporter, you are the connecting link that quickly, personally, and professionally helps patients out of crisis and then supports them on the path to a stable and confident life within a government organization.
What you will do.
- You independently hold consultations where patients can receive quick, caring attention and you discover the underlying need for help.
- You combine medical knowledge with mental health care, so that every complaint is thoroughly mapped out in a few conversations and a sustainable plan is established.
- You build a bridge from temporary interventions to lasting stability, considering relapse prevention, self-help, and aftercare.
- You are the connecting factor between the patient, the general practitioner, and the mental health care chain.
- After specialist treatment, you are ready to maintain recovery.
- You maintain close contacts with fellow general practitioners, psychologists, healthcare providers, and the social domain.
- You participate in intervision, professional development, and collegial consultations through the primary healthcare companies.
- You build a strong 'social map' around your practice, ensuring that every patient receives the right care at the right time.
- Your interventions are aimed at strengthening the patient's own capabilities.
Profile
- With your medical and mental expertise, you provide care in one place, identify risks in a timely manner, and translate complex information into clear advice.
- You identify bottlenecks and make difficult information understandable for the patient.
- You are the connecting factor between the patient and the general practitioner and, if necessary, with mental health care.
- Based on your knowledge and skills, you help people become more independent and resilient step by step.
What they ask
- Relevant prior education and demonstrable work experience in mental health care.
- Post-HBO education POH GGZ.
- Registered in the national quality register POH GGZ.
- Extensive knowledge of POH GGZ relevant NHG standards.
- Knowledge of Akwa GGZ quality standards.
- Dutch nationality is required.
