A neighborhood team organization supports residents with questions regarding care, welfare, work, income, and participation. Professionals work daily to strengthen self-reliance, social cohesion, and appropriate support for residents. This position focuses on processing applications for Personal Budgets (PGB) for multiple teams within the organization.
What you will do.
- Conduct home interviews with residents
- Perform customized assessments within the Wmo (Social Support Act)
- Evaluate applications for individual guidance, work-oriented activation, and day care
- Advise on appropriate support via Care in Kind (ZIN) or a Personal Budget (PGB)
- Assess whether residents have sufficient skills to manage a PGB
- Draft customized provision plans
- Apply relevant laws, regulations, and municipal policy
- Inform residents about personal contributions, procedures, and appeal processes
- Collaborate with colleagues within different neighborhood teams
- Professionally conduct difficult conversations, dare to ask critical questions, and maintain a balance between involvement and objectivity
Profile
- Completed relevant HBO (higher professional education) degree in Social Work
- Minimum 1 year of relevant work experience
- Knowledge of assessment and/or experience with customized assessments (preferably within the Wmo)
- Knowledge of relevant laws and regulations and apply them diligently
- Stress-resistant, maintain overview, and clearly indicate boundaries
- Feel at home in a dynamic environment where policies and working methods are continuously evolving
- Possess tact, persuasiveness, and the ability to stimulate residents and colleagues
- Work carefully and accurately, follow process steps
- Clearly inform residents about personal contributions and appeal procedures
- Not shy away from difficult conversations, dare to ask questions, and maintain a balance between involvement and objectivity
- Consistently implement policy, even if it sometimes conflicts with personal conviction
