All round Wmo Consultant
You are an all-round Wmo Consultant looking for a new challenge where you can act independently and devise creative solutions. The Work, Income and Care team of a municipal organization is ready to support residents who need assistance. The team primarily deals with activities arising from the Participation Act, the municipal Minimum Income Policy, the Social Support Act (Wmo), and the Youth Act. The team consists of 45 employees.
What you will do.
- Conduct interviews for support in the areas of housing, transport, aids, household help, guidance, and day care.
- Together with the client, look for solutions within their own social network or by utilizing generally available provisions/help, informal caregivers, and general facilities.
- Independently assess and indicate a customized provision in the areas of housing, transport, aids, household help, guidance, and day care.
- Draw up an integral and objective support plan.
- Process the application for a customized provision so that the provision can be granted.
- Participate in multidisciplinary consultations, networks, and collaborations with, among others, care providers, suppliers, and other (external) partners.
Profile
You are socially engaged and feel at home in a small municipal organization that acts as a partner in society. You work client-oriented and adhere to agreements made. You work according to the intent, think integrally, and approach tasks analytically.

- Demonstrable completed education at a minimum of HBO bachelor level.
- Minimum of 2 years demonstrable work experience as a Wmo Consultant at a government institution.
- Demonstrable work experience with collaboration, case consultation, and coordination of care within the social team.
- Agreement with the stated secondment condition.

- Minimum of 4 years demonstrable work experience as a Wmo Consultant at a municipality.
- Minimum of 2 years demonstrable work experience with applications, guidance, home interviews, and collaboration with the social team.
- Demonstrable work experience with drawing up an integral and objective support plan.
- Demonstrably willing to work 3 days at the office.
- Demonstrable work experience in the areas of housing, transport, aids, household help, guidance, and day care.

Practical
- A Certificate of Conduct (VOG) will be requested.
- An Integrity Declaration will be requested.
- If personnel are made available, the contractor must be registered in the trade register as a company that makes personnel available (in accordance with the Waadi Act).
- The contractor is subject to a Social Return (SROI) obligation for awarded assignments exceeding €400,000 within the DAS of the Talentenregio.
