An organization specializing in mental support is looking for a case manager for the Suicide Prevention, Monitoring, and Aftercare (SUMONA) department. This department offers accessible support to people with suicidal thoughts, people who have attempted suicide, and/or their relatives. The SUMONA methodology will be further rolled out regionally and nationally from 2026.
What you will do.
- Provide monitoring and case management to clients.
- Conduct conversations focused on suicide prevention.
- Coordinate care and maintain contact with network partners.
- Give information, training, and presentations to professionals and other stakeholders.
- Contribute to the further development and implementation of the SUMONA methodology.
- Perform outreach work when the situation requires it.
Profile
You are an engaged and robust professional who easily makes contact and skillfully switches between proximity and professional distance. You work independently, remain calm in complex situations, and act practically and carefully.
What they ask
- Completed HBO education, for example SPV, Nursing, SPH, MWD, or Social Work.
- Experience within mental health care (GGZ), preferably outpatient.
- Affinity with suicide prevention and experience with conversations about suicidality.
- Knowledge of the social map, laws and regulations, and provisions.
- A flexible, stress-resistant, and pragmatic work attitude.
- The ability to plan independently and manage your own agenda.
- A positive Certificate of Conduct (VOG) is required.
Preferences.
- Experience with assertive community treatment (bemoeizorg).
- Access to a car for outreach work.
