As a Wmo consultant, you investigate with residents what is needed for them to continue living independently and participating in society.

You first look at primary provisions, the network, or the Social team.
If indicated care is needed, you, as a Wmo consultant, take action.
You examine all life domains of the situation and investigate what is required.

Collaboration with access staff, the Social team, fellow consultants from Youth, Work & Income, and network partners is part of your work.
You are critical, creative, and think and act broadly.

You are able to manage complex cases and collaborate within multidisciplinary networks.

The client is a public organization focused on supporting residents with various questions. They work from an integrated team that quickly helps residents and promotes their self-direction, thinking from possibilities and the idea that residents have as much control over their lives as possible.

What you do

You conduct home visits and kitchen table discussions, gathering information and providing advice on other social schemes and possibilities.
You thoroughly and broadly investigate and draft a research report on what is needed.
You clearly motivate decisions and write official decrees for applicants.
You commission care providers and partners to implement specific Wmo support.
You drive results and monitor the quality of support, adjusting indications if necessary.
In complex or multiple situations, you manage cases of implemented Wmo support and ensure coherence and coordination among all involved parties.
You weigh risks, make professional assessments, and dare to make choices, even challenging ones.
You maintain contacts with internal and external partners, including care organizations, general practitioners, occupational therapists, and housing associations.
You seek the most adequate, appropriate, and cost-conscious solution.
You diligently record research findings, reports, and decrees in a client registration system.

What they ask

Demonstrable completed education at a minimum of HBO bachelor level.
Minimum of 3 years of demonstrable work experience in the past 5 years in the role of Wmo consultant at a municipal organization.
Demonstrable work experience with indication setting for guidance and day care.
Demonstrable work experience with laws and regulations concerning the Wmo.
Demonstrable knowledge of WLZ, Health Insurance Act, and/or Protected Living.
Candidate agrees to the stated secondment condition.

Preferences.

Demonstrable completed education in Social Work, Social Legal Services, and/or Social Pedagogical Assistance at a minimum of HBO bachelor level.
Demonstrable work experience with a specific client registration system.
Demonstrable work experience with a specific protocol.
Demonstrable work experience with the target group of mildly intellectually disabled and psychiatric disorders.
Demonstrable work experience with case management in complex cases.
Demonstrable work experience at a municipal organization affiliated with a regional collaboration.
