Coordinator Inpatient Care Pathway

A healthcare organization is a key player in the healthcare sector in the Liège basin. The institution comprises over 50 operating sites and has over 2,000 beds, covering three major sectors: mental health, geriatrics, psychogeriatrics, rehabilitation and palliative care, and support for the elderly. Under the hierarchical authority of the Director of the nursing department, the coordinator ensures the coordination of the care pathway for inpatients, promoting the continuity, coherence, and fluidity of their journey within the care and justice network. He/she acts as an interface between the stakeholders involved in the inpatient care pathway, both within the institution and with external partners and the various judicial authorities.

What you will do.

  • Coordinate the inpatient care pathway
  • Ensure the coordination of the care pathway for inpatients in conjunction with the various stakeholders concerned.
  • Promote the continuity and coherence of the care pathway and facilitate the circulation of necessary information.
  • Participate in network meetings related to the Inpatient Care Pathway (TSI).
  • Maintain collaborations with partners in the healthcare and justice network.
  • Represent the TSI in reception areas and with relevant partners.
  • Ensure interface with judicial authorities
  • Liaise with the various judicial authorities concerned by the care pathway of interned persons.
  • Ensure necessary contacts with social defense establishments, penitentiary establishments, and other relevant partners.
  • Respond to requests from the justice system within the scope of its missions.
  • Participate in network consultations and initiatives within its field of activity.
  • Organize and coordinate applications and admissions
  • Receive applications addressed to the TSI and ensure their orientation to the relevant contacts, primarily the head nurse, service doctor, etc.
  • Participate in pre-admission and admission interviews.
  • Coordinate the various steps related to the application, pre-admission, and admission process.
  • Liaise with partners involved in applications.
  • Contribute to the alignment between admission requests and the reception capacities of the TSI.
  • Monitor activity and reception capacities
  • Monitor the activity of the TSI.
  • Collect and consolidate data necessary for activity monitoring.
  • Ensure the updating of dashboards according to defined periodicities, notably monthly and quarterly.
  • Monitor the occupancy rate of available places and contribute to its optimization.
  • Draft the annual activity report of the TSI.
  • Inform and support teams
  • Organize and/or provide information sessions for teams concerning internment and the inpatient care pathway.
  • Provide necessary information for understanding the functioning of the inpatient care pathway and its main actors.
  • Contribute to the optimal care of patients.
  • Contribute to institutional projects
  • Contribute to reflection on institutional projects related to the field of activity.
  • Participate in the implementation of institutional projects entrusted within the framework of the missions.
  • Align contributions with the institution's vision, values, and orientations.

What they ask

  • Master's degree with a social focus and/or related to the function (psychology, public health, criminology, etc.) and 5 years of relevant experience.
  • Good knowledge of the Trajet de Soins Internés (TSI), its functioning, and its various actors.
  • Knowledge of the main mechanisms, partners, and networks in the healthcare and justice sectors related to internment.
  • Proficiency in the specific procedures and operating modalities of the TSI.
  • Ability to collect, structure, analyze, and utilize activity data and to use dashboards.
  • Excellent organizational and information coordination skills, including when information comes from numerous interlocutors.
  • Proficiency in the IT tools necessary for the exercise of the function.
  • Comfortable in a multidisciplinary and intersectoral environment and able to work with professionals of varied profiles and responsibilities.
  • Excellent communication skills, allowing for adaptation of discourse to different interlocutors and ensuring an interface function between teams, patients/residents, judicial authorities, and network partners.
  • Ability to identify difficulties, seek solutions with concerned actors, and determine situations requiring specific referral or consultation.
  • Demonstrate rigor, reliability, discretion, and a sense of responsibility, particularly in data management and respect for confidentiality.
  • Ability to ensure the follow-up of entrusted requests and files and to ensure compliance with institutional procedures.
  • Recognized for relational skills, ability to unite, and a spirit of collaboration.
  • Comfortable with managing complex situations and coordinating numerous interlocutors.
  • Willingness to participate in meetings, consultations, and information sessions and, if necessary, to travel to different locations as part of the missions.
  • Adherence to the institution's values, vision, and institutional project.
