Care Coordinator and Intensive Case Manager

An experienced professional familiar with a specific domestic violence measure. With oversight, decisiveness, and knowledge of the social support network, you effectively support clients, together with chain partners.
What you will do.
- You take immediate action as soon as a domestic exclusion order has been issued.
- Together with colleagues and chain partners such as child protection services, probation services, and youth protection or a similar agency, you work towards one goal: restoring safety and organizing appropriate help.
- You maintain intensive contact with the police, public health services, and the municipality.
- You quickly and sharply assess the situation through conversations with the remaining resident(s) and the excluded resident(s).
- You analyze the problems, needs, and motivation, conduct home visits, refer, and arrange shelter where necessary.
- You work systemically, always from a crisis situation and within a strict legal framework.
- You draw up care advice for municipalities and create a concrete assistance plan together with clients and chain partners.
- You contribute to sustainable safety, self-reliance, and stopping domestic violence.
- You remain an involved point of contact even after the domestic exclusion order and, together with local aid workers, try to uncover underlying patterns and prevent recidivism.
- You are constantly on the move, liaise with various parties, and primarily work with people.

- You are a proactive individual with a clear overview.
- In crisis situations, you remain calm and know how to engage people.
- You work independently, take initiative and are ready to assist colleagues when your expertise is needed.
- You value moments of reflection and connection with the team.
- Weekend shifts (1x every 7 weeks) are not an issue for you.
- You follow developments in your field and clearly translate requests for help into a work plan.
- You are a strong networker, set clear priorities, and manage your boundaries.
- Planning and organizing come naturally to you, as does switching between diverse tasks.
- You communicate clearly, handle resistance firmly, and write clear reports.
What they ask
- A completed relevant HBO education (e.g., Social Work or SJD)
- Knowledge of the law concerning the specific domestic violence measure
- Good knowledge of the social support network in the region is a plus
- Basic knowledge of socio-scientific and systemic theories
- Relevant work experience, including crisis intervention and systemic work
- Providing a Certificate of Conduct (VOG) is part of the procedure.
