Nurse

As a Home Care Nurse, join a team at a healthcare organization. The team is ready for you, and together you will find a solution for every challenge. What you will do.

  • Support and guide clients in their own living environment.
  • Truly get to know clients and provide care that suits them.
  • Strengthen clients' self-management and consider what clients can and want to do themselves.
  • Work based on a pre-established care plan.
  • Monitor and evaluate care.
  • Implement improvements, possibly in collaboration with colleagues, informal caregivers, general practitioners, and other partners in the neighborhood.
  • Participate in the on-call duty roster as a care assistant (IG) and nurse.
  • Be the first point of contact for the entire island.
  • Receive additional training to perform technical nursing procedures in the home situation.

Profile

  • Frequent contact with clients, family, colleagues, and external healthcare professionals.
  • Empathize and adapt communication to the situation and person.
  • Always be alert to changes in client behavior.
  • Enthusiastically guide interns and students.

Practical

  • The roster is built three months in advance, taking everyone's wishes into account.
  • You will work morning, afternoon, night, and weekend shifts, and occasionally on holidays.
  • In case of illness or other circumstances, the team resolves this internally, or an external temporary worker is deployed.
  • The roster is made one month in advance.
  • One day in advance, you will know which clients you will visit (your route).
  • In the morning, you check if anything has changed before you set off.
  • Client files or care plans can be read at the client's home or before entering.
  • You travel by bicycle or your own car, depending on the distances.
  • District nursing is divided into two routes: one focused on clients with dementia and a nursing/care team for clients with somatic care needs.
  • At the end of the shift, there is a short weekly meeting to share experiences, brainstorm with colleagues, and discuss case studies.
  • Team activities are regularly organized, such as a Friday afternoon drink or lunch for a work meeting.
  • Weekly sports activities with intramural and extramural colleagues.
  • Annual team outings.
