The accompaniment mission of an organization is responsible for the follow-up of pregnancies, young children, and families. This is achieved through the implementation of specific structures (prenatal consultations, child consultations, sanitary vehicles) within which medico-social follow-up and parental support are provided. Child-Parent Partners (PEP's) also conduct home visits to families who wish it.
What you do
- Establish a relationship of trust with future parents, the child, and their family
- Take note of information concerning beneficiaries and carry out and/or participate in psycho-medico-social anamneses
- Collect data from families for the Medico-Social Database
- Ensure pre/postnatal liaison, particularly through the transmission of birth notifications
- Participate in meetings in hospital structures (neonatal, prenatal, liaison), especially concerning difficult situations
- Participate in setting up the conditions required for the return home of the child hospitalized in maternity and/or in the neonatal service
- Prepare and carry out home visits and/or contacts with beneficiaries
- Observe the child in their environment (family, cultural, etc.) and in their relationship with their family
- Ensure continuity between your home tasks and actions in pre/postnatal structures
- Participate in the organization of consultations with relevant stakeholders, including in certain reception environments, refugee centers, etc.
- Organize the logistics and follow-up of visual screening sessions
- Identify situations of family vulnerability in order to propose reinforced follow-up
- Participate in consultation meetings, municipal consultations, social coordinations, etc.
- Develop and maintain collaborations with internal and external partners of the psycho-medico-social network
- Develop and implement, in collaboration with the relevant partners, health/parenting projects, municipal accompaniment projects, and annual activity programs
- Participate in the implementation of awareness campaigns and mandatory programs as defined by the organization
Profile
- You hold a graduate/bachelor's degree in General Care Nursing, Midwifery, Social Work, Assistant in Psychology (clinical psychology or psychopedagogy and psychomotricity)
- Useful experience (professional or internship) in early childhood, family support, or in the health promotion and health education sectors is an asset
- You apply your knowledge related to medico-social matters: community health, health education/promotion, public health, early childhood, pregnancy, and parental support
- You work transparently with families and demonstrate equity
- You provide advice to your interlocutors
- You respect professional secrecy and know the rules of shared professional secrecy
- Sociable by nature, you can work both in a team and autonomously
- You act proactively
- You have excellent writing skills and are comfortable with Office suite tools
- You hold a B driving license and/or a means of transport allowing easy and quick access to the workplace(s)

- Gross monthly salary of 3143.85 euros based on the current index (0 years of seniority) - Salary scale 250/3
