Youth Consultant Case Manager
For a municipal organization, an experienced and decisive professional is sought for youth and family case management. The activities focus on children and young people up to a maximum of 23 years. In addition to single requests for help, complex cases and safety issues can also be part of the work. The professional broadly maps the situation of children, young people, and families and, together with the family and the involved network, looks for appropriate solutions.
Where possible, the family's own capabilities, network, and social base are utilized. Where additional support is needed, appropriate help is organized. The professional works from their own expertise and makes independent professional judgments about the approach to cases, taking into account applicable laws and regulations and municipal frameworks. The deployed support is coordinated, progress is monitored, and adjustments are made with involved parties where necessary.
What you will do.
- Handle requests for help and cases concerning children and young people up to a maximum of 23 years
- Analyze youth and family problems and identify possibilities and support needs
- Conduct conversations with children, young people, parents, and stakeholders and develop an appropriate approach
- Collaborate with the family and the involved network on solutions and, where possible, stimulate self-reliance and the network
- Engage care providers, schools, and other involved parties where necessary
- Coordinate the deployed support and monitor progress
- Evaluate the provided support and adjust the approach where necessary
- Carefully document progress and agreements made
- Advise and inform children, young people, parents, schools, and care providers about appropriate support and the further process
- Be aware of safety issues and know how to determine appropriate follow-up steps in relevant situations
- Collaborate with relevant chain partners, including parties in the field of youth protection and safety

Profile
- Completed a relevant HBO education (e.g., Social Work, SPH, or Pedagogy)
- Extensive experience with youth and family case management
- Knowledge of relevant laws and regulations, including the Youth Act (Jeugdwet)
- Strong analytical skills and good conversational skills
- Able to make professional judgments independently
- Accustomed to weighing different interests and perspectives
- Experience with complex and crisis situations
- Knowledge of domestic violence and child abuse
- Experience with methodologies such as Signs of Safety, Phase-oriented help in unsafe situations, JIM, or Social Network Strengthening
- Able to collaborate well with various parties and disciplines
- Proficiency in the Dutch language

The social team consists of various fields, including Youth, WMO, Social Base/Social Work, and Income Support. Within the youth team, different expertises are combined to support children, young people, and families as effectively as possible.
