Care Consultant

• You conduct intake interviews with clients and/or their families to determine care needs.
• You create a care plan together with the client and/or their family and ensure the digital care file is completed.
• You select a suitable care provider, in consultation with the client and/or their family and back-office colleagues.
• You maintain contact with the client and/or their family and regularly evaluate the agreed services with them and the care provider.
• You support and coach the care provider in carrying out their duties.
• You provide, solicited and unsolicited, your opinion and advice on current developments in healthcare to your supervisor.

• Independent and proactive management of your own client portfolio.
• Socially skilled, with a degree of persuasiveness in contact with clients, family, and care providers.
• Clear communication, both verbal and written.
• Solution-oriented, stress-resistant, and creative.
• Caring and enjoying interaction with – sometimes vulnerable – clients.
• Punctual and meticulous in adhering to agreements and client administration.
• No 9-to-5 mentality.
• Speaks Dutch, and also English and/or German (working language for foreign care providers).

What they ask

• Relevant healthcare education at MBO or HBO level (preferably a nurse or VIG’er).
• In possession of a driving license.
• Computer literate.

• An independent position with ample room for personal initiatives.
• A dynamic work environment where openness and transparency are valued.
• A salary indication, depending on knowledge and experience, according to the CAO VVT, FWG 45 or FWG 50.
• A one-year contract, with the intention to extend it afterwards.
• Flexible work scheduling in your own region.
