Hospital Pharmacist Outpatient Pharmacy

An academic medical center is integrating outpatient and clinical pharmaceutical care into a single generic care pathway. A hospital pharmacist is sought to take responsibility within a process in the outpatient pharmacy and participate in day shifts.

What you will do.

• Be the pharmaceutical point of contact for prescribers within your area of focus.
• Guide patients during initial dispensing and complex therapy.
• Assess and monitor pharmacotherapy.
• Be involved in medication transfer between clinic, outpatient clinic, and home situation.
• Participate in day shifts in the outpatient pharmacy, clinical day shifts, and the on-call service of the outpatient pharmacy.
• Actively contribute to the integration of outpatient and clinical pharmacy and the innovation of the logistical concept.
• Contribute to the training of pharmacists in training and students.

• Registration as a hospital pharmacist.
• Interest in pharmaceutical care that extends beyond the hospital door: home treatment, hospital-displaced care, transmural transfer.
• Ability to work in a changing organization, working towards an integrated pharmacy.
• Eye for efficient pharmaceutical care and understanding of the connection between quality, patient care, and financial sustainability.
• Experience with outpatient pharmacy is welcome but not a prerequisite; additional training and an individualized onboarding program are offered.
