Contribute to the well-being of clients by enabling them to stay at home as long as possible, despite their care needs.
- Collaborate with family and friends to form a supportive care circle around clients.
What you do
- Provide person-centered care as a guest in clients' homes.
- Be (co-)responsible for daily care.
- Offer a listening ear and extra attention.
- Work closely with informal caregivers and volunteers.
- Build and maintain good contacts with clients' families.
- Be part of a multidisciplinary team of VIG nurses, district nurses, and various therapists.
Profile
- Completed a home care assistant (plus) training, or currently in training.
- Being a general care nurse (VIG) or registered nurse is also welcome.
- Possess an (old) care diploma and the desire to re-enter the care sector.
