You will contribute to future-oriented, network-focused mental healthcare within specific teams of a healthcare organization. The goal is to contribute to the recovery of clients. From a recovery-oriented vision, you help clients participate as independently and meaningfully as possible in society. Ambulatory care is bundled within these teams. You will learn to work with a broad population and can contribute to ongoing content development.
You will work with a dedicated team to treat people with diverse and often multiple psychiatric problems. From a recovery-oriented vision, you help people in their home situation or on location to shape their lives as independently and meaningfully as possible.
What you will do.
- Conduct recovery-oriented intake interviews to gain an understanding of the client and their request for help
- Develop, implement, and evaluate recovery plans
- Ensure care coordination within your caseload
- Maintain contact with colleagues, relatives, and network partners
- Ensure complete and transferable documentation and reporting
- Actively contribute to team development and the ongoing content development of the teams
Shared decision-making is central. You will work with clients and their network on recovery goals and carefully coordinate care, both within and outside mental healthcare services.

You are a committed professional who is resilient and energized by ambulatory work in a complex and sometimes unpredictable setting. You can handle dynamism and flexibly switch between different roles. You endorse the vision of recovery-oriented work and are naturally someone who can work well in a developing environment. We are looking for someone with a proactive, positive, flexible, and transparent attitude. In addition, you possess:
- A diploma in HBO Nursing (Hbo-V) or Social Work
- Preferably experience in ambulatory mental healthcare
- Experience with depot medication is a plus
