Social Worker

• Social research and advice: You conduct social research and propose appropriate services, tailored to the patient and their context.
• Discharge guidance: You guide patients towards a feasible discharge situation and help arrange necessary follow-up care.
• Psychosocial support: You promote the psychosocial well-being of patients and collaborate multidisciplinarily.
• Social reporting: You prepare clear social reports and advice, also for external bodies.
• Network and collaboration: You maintain contacts with external aid workers and organizations for referrals and collaboration.
• Case follow-up: You coordinate individual cases and guide patients and their context in socio-community care.

What they ask

• Diploma: You have a bachelor's degree in Social Work or Social Nursing and are familiar with the field of assistance.
• Social expertise: You are knowledgeable about social provisions, rights research, and social legislation, and have an interest in ethical issues.
• Mental health care: You have an interest in mental health conditions and wish to deepen your knowledge in this area.
• Digitally skilled: You have a good basic knowledge of Microsoft Office and other IT applications.
• Communicative: You communicate excellently in Dutch and possess good intercultural and administrative skills.
• Independent and structured: You work autonomously, set priorities, and approach problems in a solution-oriented manner.
