A healthcare organization is looking for a social worker for a part-time (90%) replacement contract. You will be part of a small but ambitious community where quality care is a top priority.
What you will do.
- From the social service, provide support and guidance to patients and families with psychosocial problems: including preparation for discharge from the healthcare institution, practical and administrative assistance, and emotional support.
- Contribute to quality care delivery within the department and help achieve agreed objectives.
- Provide integrated care based on the individual needs and questions of patients to maintain, restore, or improve their health and well-being and promote their autonomy.
- Center the care process around the patient and consult with colleagues and doctors.
- Collaborate with a multidisciplinary team responsible for the patient's total care.
Your profile.
- Possess a bachelor's degree in social work.
- Experience in the field of oncology and palliative care is a strong asset.
- Be a true mediator and possess great empathy.
- Be highly motivated and flexible.
- Be able to work well in a team and take responsibility.
- Have a sense of efficiency and quality care.
