Social Assistant

• Support individuals and organize their social reintegration:
• Welcome individuals and their families upon their arrival at the institution and conduct various interviews (pre-admission and admission).
• Inform about the unit's functioning and, if necessary, show the facilities.
• Establish or re-establish individuals' social rights whenever necessary, prioritizing their autonomy.
• Research services, structures, and support networks for individuals and accompany them in their external procedures.
• Provide individuals with avenues for reflection (e.g., the most suitable solutions).
• Ensure the link between individuals, their families, and the institution and/or administrative and legal structures.
• Protect individuals' interests regarding social security, taxes, housing, etc.
• Inform individuals about legislation and internal and external resources and help them complete necessary procedures.
• Evaluate the individual's overall situation and exchange information with various administrative, judicial, and social bodies, as well as various centers and services, respecting the ethical and legal framework.
• Provide psychosocial support to individuals:
• Ensure availability to help individuals and their families.
• Conduct interviews with individuals and their families within the institution or at home.
• Intervene in crisis situations.
• Support individuals during the pre- and/or post-treatment period and upon their return to their families.
• Continuously evaluate the individual's social situation.
• Continuously monitor the individual's therapeutic progress and, if necessary, that of their family.
• Actively participate in multidisciplinary team meetings concerning individuals.
• Participate in developing the therapeutic project with the individual and concretely organize the implementation of existing referrals and assistance if necessary.
• Organize and/or participate in therapeutic activities for groups.
• Establish individuals' social files:
• Gather data related to individuals' situations through various means.
• Establish and keep the individual's social file up to date.
• Maintain contact with various external bodies (e.g., public bodies, mutual insurance companies, employers, lawyers) and report to them, in writing or orally.
• Participate in various internal and external meetings:
• Initiate team meetings as needed and actively participate in them.
• Collaborate with other social assistants, participate in social service meetings, and apply decisions made.
• Consult on more complex problems or other issues related to individuals within the framework of multidisciplinary work consultation.
• Stay informed about the evolution of the network and matters in their field, through various working groups and training.
