Ambulatory Individual Support Worker Mixed Living

In Mixed Living, residents consciously choose to live together, meet each other, and contribute to a strong community. As an ambulatory support worker, you support residents in their development towards independence and help build a living environment where people look out for each other.
What you do
As an ambulatory support worker, you support residents who have transitioned from social shelters or a living group to their own homes. You also help build a living environment where residents know each other, meet, and help each other. In addition to individual support, you are part of a Community Team, where residents, a housing corporation, and care organizations collaborate on a strong community. You work from an ambulatory team.
There is an office in a nearby city, but you are mainly on the road visiting residents.
- You support residents with living skills.
- You help residents take steps towards day activities, work, or strengthening their social network.
- You coordinate with involved collaboration partners.
- Within the Community Team, you contribute to the further development of the housing project.
- You guide adults who have transitioned from social shelters or a living group to their own homes.
- You support them at home with independent living, participation, daily activities, network building, and practical questions.
- You guide residents in a trajectory aimed at increasing independence, with more intensive support in the initial phase and gradual reduction.
- You actively contribute to the Mixed Living concept, stimulate encounters between residents, support activities, and help create a living environment where people can find and support each other.

- An HBO bachelor's or Associate Degree diploma in Social Work or a comparable education in care and welfare.
- Experience in ambulatory support for adults with multiple problems.
- Experience with or knowledge of Mixed Living, or the desire to develop further in this area through a specialized training course.
- Experience in guiding people who transition from social shelters, protected living, or a living group to independent living.
- Possession of a car is a plus.
Practical
You will join a team of currently five colleagues. The team is growing due to regional developments and combines years of experience with enthusiasm for innovation. In your daily work, you work largely independently, for example during home visits and conversations with residents. At the same time, colleagues are easily accessible to brainstorm, discuss questions, or look for solutions together. There is hard work, but colleagues regularly meet for consultations or a shared lunch.
This combination of independence, commitment, and collaboration makes the team strong.
