Guiding clients and their relatives from the moment there are signs or suspicions of dementia.
- Providing peace, overview, and direction during an often complex and emotional period.
- The goal is for people with dementia to be able to live at home as long as possible in an appropriate and safe manner, while maintaining their quality of life.
- Operating within the client's network and collaborating closely with informal caregivers, general practitioners, district nursing, municipalities, and other healthcare professionals.
- Maintaining the common thread, ensuring coordination, and deploying the right care at the right time.
- Actively contributing to further strengthening dementia case management within a healthcare organization.
What you do
- Coordinating and managing care and support for clients with dementia and their relatives.
- Guiding from the first suspicion of dementia through to later stages of the disease process.
- Drawing up, monitoring, and adjusting coherent care and support plans.
- Advising and supporting informal caregivers, including in complex or crisis situations.
- Collaborating with chain partners and ensuring good coordination within the care chain.
- Contributing ideas for innovation and further development of case management within a healthcare organization.
What they ask
- A completed HBO (higher professional education) degree in Nursing or Social Work and willingness to follow the post-HBO training in Dementia Case Management.
- Knowledge of dementia, psychogeriatrics, and gerontopsychiatry.
- Experience with or affinity for this target group and working within a healthcare network.
- Knowledge of laws and regulations (Wmo, WLZ, Zvw) and the social map.
- Strong communication skills and the ability to switch between different levels.
- Flexible, enthusiastic, and stress-resistant.
- In possession of a driving license and a car.
- A valid Certificate of Good Conduct (VOG) is part of the appointment procedure.
